Kauai physical therapy by Hawaii Sports and Balance Center is located in Kauai, with dedicated staff members at both locations, Lihue and Kapa'a. Our offices are centrally located and easily accessible from all parts of the island for physical therapy Kauai pain relief.

Before therapy, I was evaluated by Dr. Meynard Enriquez who was highly recommended by my PCP . This was the most thorough assessment I ever had. Dr. Enriquez took note of my age, 80, my COPD condition, and a painful right shoulder that had a complete joint implantation. These were complicating factors for treatment of a decade long, severe lack of balance caused by Lipitor and subsequent peripheral neuropathy. He then prepared a treatment plan that incorporated these findings.

The plan was executed by an associate therapist, Mr. Christian Cadawas-Tanicala, who is extremely professional, punctual, patient, and reassuring. Other therapists at HSBC treat disciplines of Physical Therapy including: aquarobics, vestibular disorders, orthopedics, neurological disorders, etc. HSBC has modern, specialized equipment for different types of disorders. With these specialists and equipment, Dr. Enriquez tailors programs to meet a variety of different patients.

In my case, I am treated, at home and in my pool, as needed, by the therapist in a mobile van with a variety of equipment. I am making progress, but must continue treatment to maintain mobility as long as possible.

Meynard V Enriquez PT, DPT, CCVT, CMP • Clinical Director

Meynard is currently the clinical director at Hawaii Sports and Balance Center.  He graduated from California State University in Northridge with a Bachelor in Health Science in 1999 and a Masters in Physical Therapy in 2002.  He continued his education at A.T. Still University, earning a Post-professional Doctor of Physical Therapy degree in 2015.  He has had a mentorship for spinal cord injury at Craig Hospital and is one of only eight Certified Mulligan Practitioners (CMP) in Hawaii.  Meynard is recognized by the American Physical Therapy Association as a Certified Clinical Vestibular Therapist (CCVT) through Emory Medical School in Atlanta, Georgia, and 360 Neuro in Austin, Texas. Meynard is trained in advanced spinal manipulation through the North American Institute of Orthopedic Manual Physical Therapy.

Meynard started his career working inpatient rehabilitation at Valley Presbyterian hospital and the Southern California Orthopedic Institute.  From there, he worked at Huntington Rehabilitation in Pasadena, California, where he had the opportunity to work closely with professional soccer players from Chivas USA and Mexico, with an emphasis on outpatient orthopedics and ACL rehabilitation.  He later planned and coordinated the center’s outpatient balance and vestibular clinic, while concurrently working in pediatric home health, specializing in early intervention.  In 2005, Meynard moved to Kauai and started working at Wilcox Hospital’s outpatient and inpatient departments, and was the first to introduce specialized vestibular rehabiliitation to the island.  

In 2008, Meynard started Hawaii Sports and Balance Center with the mission of bringing new technology, evidence based physical therapy, and a new level of hands on care to the medically underserved community of Kauai.  Meynard exemplifies service to his community whether through organizing fall prevention and educational events for the Parkinson’s Support Group, the MS Society and the Agency for Elderly affairs, or injury prevention clinics at the Kauai Marathon.  He has also served 2 years as the strength and conditioning coach for the Kauai High school basketball team.  

Meynard hopes to be able to continue to give back to his community, serve all his patients, and create opportunities for his team at Hawaii Sports and Balance Center who allow him to do what he loves every single day. Meynard is a clinical instructor for doctoral physical therapy programs at Regis University, the University of St. Augustine, and the University of Mary, North Dakota, as well as the Regis University Orthopedic Residency. He has instructed in advanced spinal manipulation at the Doctoral level and enjoys mentoring future physical therapists in the profession. He has most recently been chosen to represent the Hawaii Chapter of the American Physical Therapy Association as the Centennial Scholar, a distinction given to 100 physical therapists nationwide who have been individually selected to lead the profession for the next few years.

On his spare time, Meynard enjoys fishing, bow hunting, playing tennis and spending time with his family and children.   

Jason Luke PT, DPT, CMP • Lead Physical Therapist

Jason Luke graduated from the University of St. Augustine for Health Sciences in San Diego, CA with a Doctorate in Physical Therapy in 2012, and his bachelors degree in Exercise Science from Pacific University in 2008. He has completed 4 years of coursework in the Mulligan Treatment Concepts and became one of only 8 certified Mulligan practitioners (CMP) in 2018. 

Jason enjoys working with various orthopedic injuries and utilizes the Mulligan mobilization with movement techniques, manual therapy, and patient education to help his patients regain pain free movement.  He has a special interest in physical therapy telehealth.  On his free time, Jason enjoys fishing, diving, surfing, hunting, and golf. 

Christine Reber PT, OMPT • Lead Physical Therapist

Chris received her Bachelor of Science Degree in 1990 from Saint Mary’s College of California. While there, she worked 3 years as a student athletic trainer.  In 1993 Chris graduated from the University of the Pacific with a Master of Science in Physical Therapy.   Chris then graduated from the Kaiser Hayward PT residency  in 1998. She went on to finish her fellowship in orthopedic manual therapy, and was once recognized as a fellow by American Academy of Orthopedic Manual Physical Therapists.  With her extensive training in orthopedic manual therapy, she then opened her own outpatient clinic in Aptos CA which ran from 1998-2013.  She and her family moved to Kauai in 2013.  She currently works at HSBC where she enjoys treating the spine, chronic pain and other orthopedic related injuries.  

Alyssa Friedberg PT, DPT • Staff Physical Therapist

Alyssa graduated with a Health Sciences degree in 2010 and a Doctorate in Physical Therapy in 2012 from Ithaca College. She then moved to the pacific northwest for a few years and eventually settled down on Kauai. She has a strong interest in treating the patient as a whole instead of just one body part. She enjoys working with a diverse population including pediatrics. Currently she is pursuing a certification in visceral manipulation which helps to find the root of the problem that is contributing to your pain.  She is also pursuing a certification in canine rehabilitation to help our injured companions get back to feeling their best too.

Outside of work, Alyssa enjoys volunteering at Kauai Humane Society, she is assistant coach in wrestling at Kauai High School, MMA, art projects, and enjoying everything that Kauai has to offer outdoors. 

Kaylen Wakumoto PT, DPT • Staff Physical Therapist

Kaylen Wakumoto was born and raised on the island of Kauai.  He graduated from the University of Hawaii with a BS in Kinesiology and Rehabilitation Sciences in May of 2013.  He then attended Chapman University’s DPT program and graduated in August of 2016 with an award of academic excellence.   Kaylen is certified in the Mulligan Concept for the lower quarter, LSVT BIG, and IASTM with the Fibroblaster.  He has a special interest in working with sports injuries, post-operative patients, and the geriatric population.  Kaylen’s hobbies include playing baseball, keeping his vehicles clean, and experiencing the world through food.  Kaylen plans to further his education with continuing competency courses in orthopedic and neurologic rehabilitation.

Tiffany Ikeda-Simao PT, DPT • Staff Physical Therapist

Tiffany Ikeda-Simao was born and raised on the island of Kauai.  She is a graduate of Kauai High School and received her BS in Kinesiology and Rehabilitation Sciences from the University of Hawaii at Manoa in 2013.  She later attended Chapman University in Orange, California where she graduated with her DPT degree in August of 2017.  She returned home and began working for Hawaii Sports and Balance Center in November of 2017.  Tiffany is a certified LSVT BIG and Fibroblaster level 1 IASTM practitioner.  Tiffany enjoys working with a variety of patients, from pediatrics to geriatrics.  Her hobbies include sports, going to the beach, and traveling the world. 

Christian Cadawas-Tanicala PTA • Physical Therapist Assistant

Christian Cadawas-Tanicala is an experienced Licensed Physical Therapist Assistant who earned his Associates in applied science degree from Kapi’olani Community College in 2015. Christian has obtained experience with outpatient orthopedics, acute and sub-acute care. He also has a certification in LSVT BIG providing care for patients with Parkinsons.

Christian enjoys to helping people with a wide range of orthopedic injuries, including sports and postoperative rehabilitation. He also has a special interest with aqua therapy, and mobile therapy. He attempts to provide patients with the most effective treatment using evidence-based approach in order to facilitate quick recovery and achieve patient’s goals with therapy. He is passionate about this field of physical therapy and excited to be with Hawaii Sports and Balance Center’s team and looks forward to growing with the company.

When not working he enjoys to spend time farming, fishing, and spending time with family. 

Dr. Kathryn ODonnell PT, DPT

Originally from western MA, attended University of the Sciences in Philadelphia for a 6 year doctor of physical therapy program. Started off doing travel PT in Texas, Michigan, Massachusetts, and Hawaii for 2 years in a variety of patient settings. When not working she can be found hiking, surfing, playing soccer, or performing aerial yoga. Passionate about pediatric physical therapy and currently studying for her pediatric certification.

Dr. Josh Ley PT, DPT

Joshua Ley graduated from Loma Linda University in Loma Linda, CA with a Doctorate in Physical Therapy in 2017. He was a travel therapist in Tennessee, California, and here on Kauai before joining the team. 

Joshua has a background of gymnastics, specializes in handstands, enjoys working out, hiking and traveling. He is currently working toward being a certified strength and conditioning specialist. 

Susan Mayfield PT, LMT

Susan Mayfield graduated from the University of New Mexico in Albuquerque with academic honors and has been a licensed Physical Therapist working with children and adults in the rehabilitation, outpatient, private and home care settings since 1991.  In 1996 she also became licensed in Massage Therapy to diversify her skill set in soft tissue manipulation and lymphatic drainage. 

From 1991 to 2006 she was a Clinic Manager for Rehabilitation Hospital of the Pacific and their Staff Consultant-Inservice Presenter from 1993 to 1996. In 1993 she became a Clinical Instructor and has continued mentoring students pursuing their degree in the Physical Therapy and Physical Therapy Assistant programs for more than 25 years. 

In 1992 she became certified as a Lab Examiner & Health-Fitness Instructor by the American College of Sports Medicine and in 2005 as an Industrial Therapist by ErgoScience. 

She has studied Hellerwork Myofascial Release, ErgoScience, McKenzie, Mulligan, Travell & Simons Trigger Point, Pelvic/Sacral alignment, Lymphedema, Craniosacral, Temporomandibular Joint, Strain Counterstrain, Foot and Orthotics under well respected instructors. She is continuing her studies with the North American Institute of Orthopedic Manual Therapy primarily under Erl Pettman, Founding/Senior Faculty/Chief Examiner. 

In the professional community she served as the Public Relations Chair for the Hawaii Chapter of the American Physical Therapy Association and was their Physical Therapists’ Coordinator for the Honolulu Marathon Medical Tent from 1994-1997. When she moved to Kauai in 2019, she volunteered as a Physical Therapist in the Hawaii Sports and Balance Center’s Medical Tent at the Koloa Marathon and for Wilcox’ Children Fitness Event.

Her achievements include several educational scholarships from the C.S. Wo foundation, a monetary award from Rehabilitation Hospital of the Pacific for developing a Healthy Back Aerobics program and recognition by this same organization for developing a submaximal exercise testing protocol, community fitness assessment program and a post rehab exercise program. 

Leisure activities include Chigong, Chen and Yang style Taichi, volunteer bell ringer for the Salvation Army, trying new recipes, watching HGTV for home improvement ideas and Christmas choral singing. She also enjoys traveling and going to Broadway Musicals with her husband.
